Purpose. Use this template to hand a completed project over to the service desk and the client and to record the post implementation review.
1.How to use this template
Complete the handover sections in the final week of the project and hold the handover meeting with the Service Desk Manager and the client contact. Complete the review section four weeks after go live. File the document in the project folder and link it from the Project Register.
2.What was delivered
Summarise the scope delivered against the original scope, including any change requests accepted, and list the systems and documentation produced. Record what was excluded and why.
3.Support arrangements
State how the new systems are monitored, backed up and patched, who supports them, the warranty or vendor support in place and the knowledge base articles written for the service desk.

4.Client acceptance
Record the acceptance tests completed, any open issues with owners and dates, and the client's written acceptance.
5.Post implementation review
Four weeks after go live, record whether the objectives were met, the budget and schedule result, incidents since go live, client feedback and lessons for future projects.
